Esophagitis dissecans. A case report

Introduction: It is a rare, benign, and idiopathic esophageal condition, characterized by superficial exfoliation of the esophageal mucosa, associated with certain skin diseases, mediations, injurious agents, or stem cell transplantation. Objective: To describe the disease with the existing bibliography and report a case of esophagitis dissecans diagnosed by endoscopy. Case report: A 47-year-old male, with personal history of hypertension, diabetes mellitus type 2, smoking and chronic consumption of acetilsalicilic acid; he went to a public hospital emergency room due to upper gastrointestinal bleeding, with hemodynamic instability and secondary anemia. After his stabilization, he is referred to upper gastrointestinal endoscopy, reporting desquamative esophageal mucosa on distal third, hiatal hernia and gastropathy edematous. The biopsy report squamous cells with abundant polymorphonuclears. Results: The esophagitis dissecans has an incidence of 0.03%, idiopathic etiology, without a well-established treatment, only general support an usually resolves without sequelae. Conclusions: It is a rare disease with non-specific symptoms, with difficult clinical diagnosis, so the use of endoscopy and biopsies help to its diagnose and confirm it.
